Activity: Summer Teen Rallies Dates: June 14 - August 9 every Wednesday 6:45 – 8:15 P.M.

Sponsors: Pastor Kris and other youth staff

I give permission for my child (name above) to participate in this activity and to engage in everything involved. I agree to be responsible for the expense of medical aid where not covered by the church accident insurance policy. I also understand that if my son/daughter must be sent home because of disciplinary or other problems, I will assume the additional transportation cost. I agree to release Northwest Valley Baptist Church from all liability originating from my child’s participation in this activity.

IN CASE OF MEDICAL EMERGENCY, I hereby give permission to the physician selected by the sponsor to hospitalize, secure proper treatment for, and order injection, x-ray, anesthesia or surgery for my child as named above.
